Christie, thanks! I looked at my nearest 2 Walgreens (and my Fred Meyer) and have come up empty handed. I have contacted (via e-mail) Carlo Rossi to attempt to obtain a form.
I found a MIR form in the beverage area of my Walgreens. It is a Budweiser form,but you do not have to buy any alcohol. You must spend $10.00 on items listed on the form and mail it in. You get $6.00. The items to circle on your cash register receipt are: cheese buns deli items,salty snacks etc. It is on a tear pad.
