Wording is different on this rebate form. Are we required to send in the actual UPC off the product? If so, how do you do that on a bottle of shampoo, for example?

Jennifer- I am not sure if this will work but this is what I am doing – taking a picture. On the form it says” Original UPC from the package (include a graphic if possible)” So I have taken a colleague of pictures and then cut and paste them on a piece of paper. So for instance I purchased 4 Pantene shampoos and conditioners. I took pics of the entire group of the 4 products – both front and back and then individually took pics of each UPC symbol. So I had 6 pictures on one sheet of paper and attached it to my rebate form. Like I said I am not sure it will work but I am gonna try :-) Let me know what you end up doing :-)
