If you have the e-mail you’ll have to click on “Get Reward Form”, you’ll confirm your information and an individualized form will pop up just for you. This deal is one per household and you have to have received the e-mail to participate. You’ll need to make your purchases between 07-09-2010 & 07-23-2010 and send in the form by 08-24-2010.
